“How far have we come?”
The Cullen inquiry into the Piper Alpha disaster still has much to teach us thirty years on. It revealed the early warnings and missed opportunities that might have prevented the tragedy and made a wide range of significant recommendations.
The lessons to be learned are applicable far beyond the offshore oil industry, across all hazardous industries, and every bit as relevant today, but are the lessons being properly applied or do we sometimes (wrongly) convince ourselves that our organisation has adequate precautions in place?
